Scheduling an AppointmentTo make an appointment at our either Clinton or Maquoketa offices, please contact our Clinton office at 563-243-1413 or 800-664-1413. If you must reschedule your appointment, we ask that you give our office at least 24 hours notice. We will call a day or two prior to your appointment to confirm the date and time. If you’re a new patient, please complete the self history form that we send to you prior to your appointment (it’s also available on this site under Patient Info – Forms). You should bring your completed self history form to your first visit. If you have any questions about how to complete it, please call, or one of our staff members will help you when you arrive for your appointment.
