Call our office; 563-243-1413 if you have any pregnancy or gynecological problems. During our normal clinic hours our staff will assist you with your concerns. When we are closed, your call will be routed to our answering service. They will contact the physician on call regarding your problem and you will be contacted by the physician or they will direct you based on the physician’s instructions.
At Women’s Health & Family Services, we strive to get you appointments as quickly as you need to be seen determined by your needs. We will contact you a day or two prior to your appointment to confirm the date and time.
Please print the “Authorization to Release Medical Information” form, complete it, and sign the form. It must be signed by the patient, even if the patient is a minor. We cannot accept a computerized signature. Send the completed form to Women’s Health & Family Services at 2635 Lincoln Way Suite A, Clinton, IA 52732 or fax it to 563-242-9992.

Copies of your medical records for women’s health services are needed from your previous provider. Your most recent year’s annual exam should be sent. More extensive records and medical history are needed for problem visits. Please complete the medical records release form, sign it and send it to your previous provider. Once records are received by Women’s Health & Family Services, we can schedule an appointment. If records are not received and duplicate services are performed and not covered by your insurance, you may be responsible for the charges.
Patients requesting medical records to be sent to another provider need to complete the “Authorization to Release Medical Information Form.” This includes for referrals and ongoing treatment.

Women’s Health & Family Services obstetricians are on staff at Mercy Medical Center, Clinton, IA. This is the hospital for the delivery of your baby or for your surgical procedure. Some surgical procedures may be scheduled at Gateway Surgery Center.
