Scheduling an Appointment

Scheduling an Appointment
To schedule an appointment in Clinton or Maquoketa, contact our Clinton office at 563-243-1413 or 800-664-1413. If you must reschedule your appointment, we ask that you give our office 24 hours notice. We will call a day or two prior to your appointment to confirm the date and time. Each new patient is asked to complete a Self History form that is sent to your home prior to your appointment. You should bring the completed form to your first visit. If there are questions about how to complete it, one of our staff will help you when you arrive for your appointment.

If you have health insurance, you may check with your insurance carrier before your first appointment to verify your coverage including:

1. Deductible and co-pay amounts;

2. Pre-authorization requirement, if any. When you arrive, you will meet with Shari or other billing staff, who will help you with your insurance and payment arrangements, if needed.
