Before Your Appointment

Preparing For Your Visit

Women’s Health & Family Services is committed to making your patient experience the best possible. We would like to share important information to help you prepare for your appointment and to help you understand what to expect when you arrive.

Your Insurance Coverage:

Understand your insurance benefits; what services are covered, what copays & deductibles you are responsible for, and check to see that Women’s Health & Family Services providers are considered “In-Network Providers”.

Before your appointment:

Check with Women’s Health & Family Services to make sure you have provided everything needed; your insurance policy information including ID number, your Social Security number, your current address and phone number.

What to bring to your appointment:

  • Your photo ID
  • Your health insurance card
  • Your proof of income from the last 30 days (this is an annual requirement for all patients (pay check stubs or if self-employed your last tax return)
  • Your co-payment – we are required by your insurance company to collect a co-pay if applicable. We accept cash, check, debit cards, and credit cards
  • Completed patient forms
  • Payment on account balances if applicable

Your Medications:

Your health care is important and a complete picture of what’s going on with you includes knowing what medications you are currently taking. It is important to bring a list of medications with you to your appointment.

Family Members & Friends:

Patient confidentiality and Title X guidelines require initial discussions with the patient to be private first. Then the patient can request a family member or friend to be included as part of their appointment. Contact a staff member for special circumstances.

Arrange for an Interpreter of Translator:

Communication with your provider is important. If you have a language need, then arrangements can be made to have an interpreter/translator. If you have a close friend/relative over the age of 18 that will accompany you, please make sure Women’s Health & Family Services knows who will be at your appointment. If you do not have an interpreter, we offer translation services / interpreting in many languages to help you during your appointment. This must be arranged prior to your visit. If needed, contact Women’s Health & Family Services to make arrangements prior to your appointment.
